I am requesting a Credentialing Action:
- For initial/renewal/short term credentialing action, a Credentialing Request Form must be completed and submitted to an Office of the Chief program specialist.
I am requesting a Medical License or Professional Liability Insurance Reimbursement Action:
- For a medical license reimbursement to be initiated, the requestor must provide proof of cleared payment and complete and sign/date a Medical License Reimbursement Form and provide a hard-copy to an Office of the Chief program specialist, room 2S235.
- For a professional insurance reimbursement to be initiated, the requestor must provide proof of cleared payment and complete and sign/date a Professional Liability insurance Reimbursement Form and provide a hard-copy to an Office of the Chief program specialist building 10/2S235.
I am requesting an Outside / Elective Rotation Action:
- All outside/elective rotations must be requested well in advance of an upcoming rotation. Ideally, we need 3-6 months depending on where the rotation will be in order to ensure that all necessary paperwork will be completed in advance of the rotation start date. Please review and complete the Elective Rotation Approval Form, and submit the completed form to an Office of the Chief program specialist.
I am requesting a Publication Request Action:
- For a publication request to be initiated, a Publication and Abstract Clearance Form must be completed and submitted to an Office of the Chief program specialist. A hard-copy completed with the requestor's signature is preferred and may be dropped off in Bldg 10/Room 2S235.

I am requesting a Travel Action:
- If attending a Conference, please complete and submit a Conference Request Form within 215 days from the beginning of the conference. You will also need to complete a Travel Request Form (see below) once you receive confirmation that your conference request has been entered in the system.
NOTE: For registrations paid in POTS, please include the Payment of Tuition Form to your travel planner. - For either Domestic/Foreign/Sponsored/Non-Sponsored Travel, a Travel Request Form must be completed well in advance and submitted to an Office of the Chief program specialist.
- Travel Lead Times:
a. Conferences, sponsored, domestic/foreign travel must have a travel request form completed and submitted 8 weeks in advance. NOTE: (Normally travel data calls are requested 6 months in advance, so please let your travel planner know so they may enter the conference in the system).
b. Domestic non-sponsored, non-conference travel must have a travel request form completed and submitted 8 weeks in advance. - For Clinical Rotations (residents/fellows) please complete the Travel Request Form and submit to the Office of the Chief program specialist. Please note that the TRF is not required for rotations that are under 50 miles from the NIH.
- Clinical Rotation Lead Times:
a. Rotations outside the local 50 mile radius (from NIH), you must complete a travel request form and submitted at least 8 weeks in advance of your rotation start date.
b. Rotations that are under the local 50 mile radius (from NIH), you only will need to submit parking and/or toll receipts, and a map print out showing the round-trip distance (Google maps, mapquest, etc.)
